Peer Recovery Specialist - Behavioral Health

The Peer Recovery Specialist in this position will be responsible for the following:
- Assisting Behavioral Health Home (BHH) enrollees identify and achieve their health and wellness goals.
- Providing medical and behavioral care coordination and peer recovery support to clients with SPMI and/or chronic health conditions that meet BHH program criteria
- Helping individuals recognize their capacity for recovery and resilience; uses maintenance interviewing concepts.
- Providing client transportation as needed.
- Providing or supporting BHH services including:
- Comprehensive Care management
- Care Coordination
- Health Promotion
- Comprehensive Transitional Care
- Patient and Family Support
- Referral to Community Support Services
- Collaborating with a wide range of client supports including family members and community medical providers.
- Providing skill-building and support for self management, self advocacy, health and well-being, and using community supports.
- Working to reduce health care disparities whenever possible.
- Maintaining documentation in compliance with agency and program standards.
- Participating in staff meetings, supervision, trainings and other meetings as assigned.
- Additional duties as required.
