Community Bridger

POSITION DESCRIPTION: The position of Community Bridger is based in Rocky Hill, Connecticut, with statewide travel a requirement. Community Bridger offers brief, transitional peer support to people aged eighteen and older who are currently involved or at risk of involvement in psychiatric services. Community Bridgers are not expected to be members of a treatment team nor take on the role of case management or crisis workers. Important skills and attributes applicants should bring to this position include remaining culturally and trauma responsive, gender affirming, recovery oriented, person centered, situationally aware, open-minded, excellent communication skills, a sense of humor, growth oriented, solution focused, and patience.

ESSENTIAL JOB DUTIES:

  • Model recovery with collective care, accountability, and ethics.
  • Assist the Director of PRS with documentation and reporting on behalf of the Bridger team.
  • Onsite in Rocky Hill 2 days during the regular business week.
  • Present on Bridger in the community as requested, and submit follow up documentation.
  • Provide 1:1 peer support to at least 30 people living in Connecticut, annually, and no more than 15 people, concurrently.
  • Meet with participants who are incarcerated or hospitalized, and maintaining connection following discharge.
  • Meet participants in the community and provide transportation, as needed.
  • Share about Bridger as a peer recovery community resource.
  • Develop relationships with community resources and other supports.
  • Act as an ambassador of Advocacy Unlimited and the Community Bridger program.
  • Actively promote peer recovery resources.
  • Assist participants to create recovery plans and move toward achieving goals
  • Maintain up to date and accurate records of all work-related activities.
  • Meet all internal accountability standards.
  • Adhere to all contractual obligations
  • Attend team meetings and trainings
  • Other duties as assigned.

QUALIFICATIONS:

High School diploma or equivalent
Recovery Support Specialist Certification
At least 1 year working as a Recovery Support Specialist, or equivalent position in peer recovery services Knowledge of community support systems and resources
Personal lived experience related to mental health, addiction, and/or trauma recovery

EXPERIENCE:

  • Direct experience offering support to people and community groups from diverse backgrounds
  • Motivated to advance collaborative decision making and collective accountability
  • Ability to think strategically, creatively solve problems, exercise good judgment and lead
  • High proficiency in Microsoft Office 365
  • Excellent communication skills, both written and verbal
  • Must have a clean driving record, up to date Motor Vehicle Insurance and Driver’s License
  • Must have a reliable vehicle for statewide travel
